Report of Premiums for Assessment Purposes
As of March 2, 2020 the forms for the 2019 Report of Premiums are accessible through the link below:

The reporting form categories are now based on the NAIC line items reported on the Annual Statement
– Exhibit of Premiums and Losses (Statutory page 14), Page 19.LA for Louisiana.
Click the ROP Instructions and 2021 Assessment Letter to view the available documents and instructions.
• If you are reporting for a single company – Use the Report of Premiums Single Company link.
• If you are combining two or more companies – Use the Report of Premiums Group link.
(Note – Remember to list all companies in the group in the space provided on the form.)
The due date for Report of Premium submission will be June 30, 2020.
To ensure that both PIAL and its member companies have a copy of the completed Reports of
Premiums, please follow these steps:
• Print and save the form as a PDF file to your computer.
• Submit the electronic data to PIAL via the website.
Premiums reported are verified by comparing them with your Annual Statement therefore you must
